Elder
Care Management Services
.bringing clarity to complex healthcare
needs!
At
Excel Healthcare, our geriatric care managers mission is to anticipate,
identify, and respond to the growing expectations and needs of
the older adult and their families.
Objective
Our geriatric care managers recognize that each individual requires
a unique type of care options. Each person searches for meaning
in personal life experiences and the choices and decisions made
are based on that person's individual values and beliefs.
Our
primary goal is to provide a comprehensive plan of care which
addresses all aspects of care options needed in order to assure
the safety and well being of each individual.
Who
are Geriatric Care Managers?
A geriatric care manager is a professional who specializes in
assisting individuals and their families in planning and obtaining
their long term care goals. Our staff is comprised of over 40
years of nursing expertise.
No one wants to see a loved one endure a prolonged hospital stay
or placed in a nursing home against his or her wishes. Our geriatric
care managers strive to provide individuals with options and alternatives
to nursing home placement, when appropriate, allowing them to
remain in their own homes for as long as they desire to do so.
Our
focus is to identify appropriate and safe care options for individuals
that navigate toward one common goal:
assisting each individual to achieve his/her maximum level
of health and independence as quickly and for as long as possible.
According
to the US Bureau of Census, an elderly population explosion is
inevitable between the years 2010 and 2030. This will primarily
be due to the combination of increased life expectancy in the
elderly and the onset of the baby boom generation reaching the
age 65. By the year 2030, one in every five US Citizens will be
elderly. Advanced medical research and technology are enabling
more and more individuals to withstand acute illnesses and to
live longer, healthier lives. These advancements not only benefit
the quality of life, but are also leading to more of our elderly
Americans depending on elder care assistance in the coming years.
Many
times families become overwhelmed in making appropriate care decisions.
Our geriatric care manager can assume the role of advocate for
your loved one and find optimum care arrangements. The need for
a geriatric care manager depends on your personal situation.
Our
Care Management staff has extensive expertise and knowledge about
the cost, quality, and availability of services in our community.
